Completing Your Paperwork Online Is Easy! Just follow these steps...

Part 1 - Create Your Account

• You will receive a PIN # when you schedule your first appointment.
• Check your email for your new electronic health record
• Enter name and PIN #
• Enter contact information
• Accept all release forms
• Submit your information to activate your account registration and verify your email address

Part 2 - Activate Account And Complete Paperwork

• Check your email for your patient activation
• Click the link in the email to activate your account
• Log in using your email as your username
• Complete the following forms in your patient profile located on the left side of the screen:
  • About Me
  • Health History
  • New Case Intake
